Mid-Rotation Self Assessment

The Mid-Rotation Self Assessment allows students to reflect on their progress during a rotation. This self-assessment is submitted to the preceptor who completed the Mid-Rotation Assessment Feedback Form in the Preceptor Portal and supports meaningful mid-rotation feedback discussions.


Accessing Mid-Rotation Assessments

The Mid-Rotation Assessments page displays all assessment requests associated with your current and upcoming rotations.

  • Each request includes:
    • Request Number
    • Due Date
    • Status (Current or Future)

Select a request to open and complete your self-assessment.

Once opened, complete the following sections:

  • Preceptors
    • Select the appropriate preceptor
    • Preceptors are filtered based on the rotation’s discipline
    • This should be the preceptor who is completing your mid-rotation evaluation

Self Assessment Questions

Provide thoughtful responses to the following:

  1. Progress Toward Requirements
    • Reflect on your progress in meeting rotation requirements
    • Include core conditions, procedures, observations, and teaching evaluations
    • Note any challenges or difficulties
  2. Strengths
    • Identify your strengths based on:
      • Your own reflection
      • Feedback received (verbal or through CAS)
  3. Areas for Improvement
    • Highlight specific areas you want to improve
    • Be clear and actionable
  4. Additional Comments
    • Include any other concerns or issues
    • This information is shared with the clerkship director if needed

Submitting the Assessment

  • Click Submit to complete your self-assessment
  • Click Cancel to exit without saving

Once submitted:

  • Your responses are sent to the selected preceptor
  • The self-assessment supports your mid-rotation feedback discussion

Viewing Submitted Self-Assessments & Status

Once you have submitted your Mid-Rotation Self-Assessment, you can return to the Mid-Rotation Assessments page at any time to review your submission and track its progress.


How to View Your Submission

  1. Navigate to Assessments > Mid-Rotation Assessments
  2. Locate the relevant request
  3. Click to expand the assessment

You will see:

  • Session and course details
  • Week and due date
  • Current status

Viewing Your Self-Assessment

Your submitted self-assessment appears under your name, labeled as Self-Evaluation.

  • Click on your name to expand and review your responses
  • This allows you to revisit what you submitted prior to your mid-rotation discussion

Viewing Preceptor Mid-Rotation Feedback

Below your self-assessment, you will see the selected preceptor(s).

For each preceptor, you can view:

  • Email address
  • Submission status
  • Date submitted (if completed)

Tracking Assessment Status

Each request includes a Status to help you understand progress:

  • Current – The assessment is active and awaiting completion
  • Completed (Submitted) – The preceptor has submitted their feedback
  • Pending – The preceptor has not yet completed the assessment

You can also click Workflow History to view the full timeline of the assessment, including when it was submitted and completed.
